    To register your pharmacy for Toniq Guard Electronic Controlled Drug Register, please complete the form below. Once we receive your registration, we will prepare a contract for you to sign and return to us. We will then confirm timing for onboarding your pharmacy to use Toniq Guard.

    *As part of the onboarding process for Toniq Guard, we ask customers to agree to updated Toniq terms and conditions.  You can choose either a 3-year contract term or a month-to-month contract. If you select the 3-year contract term, you will receive a 20% discount on Toniq Guard for the duration of the contract.
